At Indy Paws, we believe in building strong relationships with our clients and their furry companions right from the start. During your pet’s first visit, we take the time to get to know you both. Our experienced veterinarians will conduct a thorough examination, attentively discussing your pet’s medical history, lifestyle, and any concerns you may have. We are here to answer your questions, provide guidance on preventive care, nutrition, and ensure your pet’s overall well-being.
When you come in for your first visit, we request that you send your pet’s full medical record in advance. This allows our team to review and ensure we can provide the best recommendations for your pet. If this is not possible, you can bring your pet’s current medical record and a list of any concerns you may want to be addressed.
To fill out all of our new client information prior to your appointment, please click the ‘New Client Form’ button below. If you need to fill this form out at the hospital, please plan to arrive at least 15 minutes prior to your appointment time.
